Part 1: Initial information

Malachi attends the setting 5 days a week from 08.30 until 16.30 whilst his mother studies Health and Social Care at the local college. He has been attending for over a year, he is a sociable, well-presented little boy who is reaching all his developmental milestones. He is bright and inquisitive and eager to engage with activities. He is close to his key workers Sarah and Jean. He separates easily from his mother Kisi at the beginning of the day but is happy to see her when she collects him.

Malachi has very dry skin and eczema, this is managed by emollients prescribed by the GP. His mother applies these before he attends the setting, but Malachi also reapplies the cream to his hands after washing them. Kisi supplies the cream for Malachi to use.

Kisi is polite and pleasant to the Nursery staff, but she does not talk about her home life. When her son joined the setting, she informed them that she is no longer with Malachi’s father, but they are amicable, and Tyler sees his son every 2-3 weeks depending on his work schedule. Tyler lives in Brighton. The emergency contact for Malachi is Stella Ansah, Kisi says she is a family friend whom she met at Church. Stella has collected Malachi from the setting and he appears to have a good relationship with her.

However, Malachi does not return to the setting after the half-term break, staff call Kisi, and she reports that both are ill with the flu. She apologises for not calling in and says Malachi will return as soon as he is well enough.

When Malachi returns to the setting he is withdrawn, appears to be much thinner and looks unkempt. The eczema on his hands and arms is extensive, he cannot stop scratching and some areas appear infected. He screams when staff try to apply cream to his hands, and he won’t do it himself. He is uncharacteristically angry and even tries to bite Jean. When Kisi comes to collect Malachi, she too appears unkempt, she looks tired and when staff ask her to take her son to the GP to get treatment for the infected skin condition, she bursts into tears and says that she is struggling a bit at the moment. Jean takes her into the office to talk to her, Kisi is reluctant to say anything but eventually shares that she is in debt, has fallen behind in her college work and she feels too tired to tackle her problems. She also tells Jean that she is 26 weeks pregnant from a one-night stand and she feels very embarrassed about this. The father will not be involved.

The following Monday, when Kisi drops Malachi at nursery, she appears very low in mood and has clearly been crying. After a lot of prompting, she tells Sarah that she has no food in the house and has no money for gas and electricity. She is clearly worried about sharing her concerns, but she is worried about Malachi. Sarah asks for consent to discuss the family’s needs with the Health Visitor (HV), and she reluctantly agrees. Sarah speaks with the HV who agrees that the family needs support and she arranges to meet the family at home.

Later that week, Sarah talks to Kisi who says that the HV has made a referral into Children’s Social Care requesting a Child in Need Assessment. She feels ashamed about this but says that she does need help. Sarah tries to explore what support Kisi has but she says that her family are not local and although she has friends at college they don’t socialise outside. She says her friend Stella is supportive, but she works full time. The Church has previously offered her some financial support, but she is too ashamed to ask again.

Following the Child in Need Assessment, Children’s Social Care feel that there is no role for them, but they have sign-posted her to other services to support her; including a free debt management charity, the local food bank and a young parent’s group. They have given her a small one-off financial payment to ensure she has gas and electricity. The Social Worker has also applied to a charity to get a new washing machine for the family as the old one is inefficient, and Malachi’s clothes need to be cleaned effectively because of his skin condition. HV also suggested that Kisi speak to her GP and the midwife about her low mood.

Malachi becomes more outgoing and sociable again in the following few weeks and Kisi reports that she is getting back on top of things, including her college work and she has started getting things ready for the new baby.
Unfortunately, 5 weeks Malachi’s attendance at the setting becomes erratic. When he does attend, his clothes are dirty and his skin is itchy and sore, he is less sociable and lashes out at staff and other children. Kisi does not stay to chat at the end of the day as she did before and becomes very angry when a staff member reminds her that Malachi has no skin cream in his bag and that the skin on his hands looks infected again.

Part 3.
Malachi comes into the setting one morning, he is limping, he has obviously been crying. Kisi reports that he tripped over on his way to nursery. She says he has a small bump on the knee but there is nothing to worry about, Sarah asks if she can look at his leg but Kisi says there’s nothing to see. Kisi is wearing heavy make up which is very unusual for her. Later that morning Malachi is obviously in pain and begins to cry, Kisi cannot be contacted by phone. Sarah and Jean take Malachi to a private area to look at his leg. It is very swollen and bruised, there is a large mark on his thigh which looks like the imprint of a belt buckle. When Jean asks him how he did this he says, “My new daddy did it, because I’m bad”.
